2012年，第37屆國際婦女泌尿學會(IUGA)於9月3日至9月8日間，假澳洲的Brisbane舉行。如同往例，在為期五天的會期裡，前兩天為workshop部分，而後三天則為研究論文的發表。此次較以往不同的是，本次的年會裡，首次舉行了Pan-Asian meeting，由來自台灣、香港、日本、大陸、菲律賓、新加坡、泰國、馬來西亞、印尼等亞洲國家的專家學者，針對包含了POP之UD診斷、mesh的使用、open/laparoscopic/robotic sacrocolpopexy，midurethral sling對USI之治療及recurrent、complication的處置，OAB、fistula、sexual function等議題，作經驗與心得的分享，不但可互相學習、交換意見，更可藉此促進亞洲各國間的友誼。

 

本次的workshop較去年少，共計有29個，其中仍以與POP相關的題目為最多，共計有10個，內容包括anatomy、pessary、recurrent POP、mesh、complication等。其中較特別的是由John DeLancey所主持的cadaver lab，藉由live dissection對anatomy作介紹，另外，搭配了由Mickey Karram所主持的live surgery，對apical prolapse、uterine preservation surgery、anterior prolpase surgery等作手術示範。尿失禁的topic僅有一個，是針對complication之evaluation and intervention。影像學部分仍以ultrasound為主，共有4個，包括2D、3D、4D超音波對POP及pelvic floor disorders的評估等。其他的題目則有obstetric tears、animal model、evidence-base urogynecology、QoL、chronic pelvic pain、intractable OAB、basic science、laparoscopic suturing等。

 

  在後三天的議程，主要以論文發表為主，總計有510篇研究論文被接受，另外包含了一個State of the art lecture、一個Ulf Ulmsten memorial lecture、一個round table 及兩場精彩的debate。此次由廠商所贊助的symposium僅有兩場，皆與mesh相關，除了介紹minimal mesh approach for anterior apical defects外，mesh相關的complication及management是另外一個重點。

 

  State of the art lecture是由澳洲當地的知名超音波大師Hans Peter Dietz主講”The role of USS in pelvic organ prolapsed surgery-The way I do it.”由ultrasound對levator trauma、avulsion、hiatal ballooning等，對POP的發生與recurrence做了精彩且扼要的介紹，另外也將ultrasound用於mesh術後的追蹤，surgical failure之探討等研究一併闡述。Ulf Ulmsten memorial lecture的題目是：teaching and learning in urogynecology，由Anthony R. Smith主講。另外，此次會議所安排的兩場debate題目分別是:” The FDA got it all wrong on transvaginal mesh”及”Pelvic floor exercise are overrated”，由來自不同國家的專家學者分成正、反雙方，提出他們對該議題的意見，同時藉由選答器讓與會聽眾參與意見，精彩的內容吸引為數不少的聽眾，在聆聽不同意見的同時，也提供機會讓我們對mesh及pelvic floor exercise重新做一番省思。Round table的主題是: Can the pelvic floor withstand vaginal delivery: Mechanics of childbirth and the pelvic floor.請到的是聞名世界的知名美國婦女泌尿解剖學大師John DeLancey主講，由vaginal birth所造成的相關pelvic floor disorders如SUI、POP、fecal incontinence、UUI等問題開始，延伸至哪些方法可減少birth related injury，如shortened the second stage、avoid instrumental delivery、CS等，進而至生產所引致之major levator ani defects，levator ani 所可能發生的muscle tearing、denervation、muscle compression等，經其研究結果發現compression對levator ani所造成的影響是日後可恢復的，所以，縮短第二產程對pelvic floor muscle的compression並無助益，而如何減少injury的發生，如pre-streching perineum等，是日後需注意的重點。此次的round table可說是會議中吸引最多聽眾的一場，幾乎座無虛席，也顯現出DeLancey其世界級大師的風範。

 

  論文發表的內容依舊涵蓋了尿失禁、骨盆腔脫垂、膀胱過動症、懷孕生產與骨盆腔功能障礙、性功能障礙、尿路動力學檢查、QoL等範圍，大多數與臨床研究及手術等相關，基礎研究相關的論文則較少。總計有50篇podium report、12篇video及138篇oral poster。

 

  本年度的最佳abstract為Corina Schmid – 2012 Cochrane Review- Surgical Management of Pelvic Organ Prolapse；最佳video為Phillip Rahmanou – Laparoscopic Hysteropexy 1- 4 years follow up of women post operation；最佳oral poster為Farah Lone – Vascularity of the Urethra in urinary continent women using colour Doppler high-frequency endovaginal ultrasonography (EVUS)；最佳podium則是Julien Labrie – Long term results of pelvic floor muscle training or midurethral sling surgery for female moderate to severe predominant stress urinary incontinence: A randomized controlled trial。
